You would think they would automate that process and eliminate any possible driver error.

That's not possible to really automated this process because the driver must not be in accelerate phase. Driver must also have a manual procedure to apply: no engine acceleration and he must cut the engine power with a switch in cab to cross the area symbolized between visible panels on the picture.
