Why do some of these GEs have the top headlights horizontally arranged at the top of the cab like this one, while others have them arranged vertically at the top of the nose? And what's that fuel-lid-on-a-chain-looking device where the latter headlights would have been? (I'm guessing it's at the request of the railroad?)

Kurt, the headlight position is just a matter of choice for the railroad. As for the chain, my guess is it is for sand.
