If the Driver/Engineer is on the starboard side, as seems to be usual on US Locos, he is on the blind side and the weakness of this design is apparent here. Yes, the car driver is an idiot but the make and design of the unit is just lacking for street running...................

The side of the photo it's the engineer seat, the other side is occupied by the "enginner helper", that's means that both sides of sight are covered.

The engineer/driver could have been a bow figurehead and not been able to stop in time. You are dealing with hundreds tons of mass here and it takes distance to stop. The train could not have been traveling much more than 8 MPH/12KMPH to have stopped in this distance. Here the fault clearly lies with the inattention or incompetence of the driver.
