What was with CSX only adding their scheme to fallen flagged locomotives to the noses first, and not just repainting the whole thing?

Ian - CSX decided that the "stealth" scheme was just that at crossings so they came up with the "yellow nose" concept. In the mid-90's, it carried over to the darker predecessor schemes such as the Seaboard System, Family Lines, SCL, B&O and C&O. The idea was to improve visibility at grade crossings.
