I would have love to have seen a night shot of the sparks this lock-up was making, bet she squealed real loud too.

Looks like a fresh wheel, might have have another look at the brakes.

AAR procedure to look at brakes with a slid flat like this. It only takes one lazy conductor to leave a handbrake on. Happens often.

Thanks for the thoughts on what caused this problem. Lying on the ballast below the wheel was a 1-ft length of steel comprised of numerous thin plates that had stuck together as they piled up on top of another as the car moved down the tracks. A small length of this steel can be seen in this photo just inside the rail.

It's called "built up tread" if it continues for a long time, say you're on a long downgrade like at Sand Patch, and those chunks can get so big that they actually cause derailments. Good thing they caught this one in time.
