Can you elaborate on the story here? How did the yellow/blue loco end up sideways?!!

The yellow blueish loco derailed, then workers figured they could clear the other tracks before any trains arrived. Stupid move. I know whenever a derailment occurs I give the order for all trains within a block of the location to STOP and STAY!
