An uncovered crown sheet is the worst thing that can happen during management of a fired-up steam engine. It may have also been the case, or a contributing factor, that the crown sheet water level gauges (usually one on each side of the cab for both the fireman and engineer to observe) were defective since reading them would indicate that the level was dangerously low. They are present expressly to prevent this kind of catastrophic failure.

With the big equipment sheds out front, is the tower still a manned location at that date? And speaking of manned, foot bridge to upper floor back door is a point of interest.

As I recall, the tower had been closed by then. The wooden walkway was connected with stairs going to track level behind the tower, and to the highway higher up on the embankment. The ICC report of the accident can be found HERE.
